  • Please provide a formal letter of intent addressed to Mr. Dennis Lorenzo B. Pinlac, CSEE – OIC, OPTICAL MEDIA BOARD, indicating the details of shipment, purpose of shipment and reason why we need to grant you a One Time Certificate.
  • List of Requirement for One Time Certification:
    • Notarized Affidavit of Undertaking – please make sure to upload the affidavit on the online portal
    • Airway Bill or Bill of Lading
    • Commercial Invoice (Import) or Pro-forma Invoice (Export)
    • Packing List
  • Submission of application shall be accepted through the OMB Portal

    • Notarized Affidavit of undertaking
    • Airway Bill or Bill of Lading
    • Certified Copy Of Commercial Invoice (import) and/or Pro-forma Invoice (Export)

  • Payment of the following Fees:
    • Import/Export Permit Fee: P500.00/Way Bill or Bill of Lading
    • Clearance Fee : P 50.00

  • A. FOR ALL APPLICANTS
  • Two (2) pieces latest photo of the applicant with signature at the side
  • Tax Identification Number (T.I.N.) of the company
  • Notarized affidavit indicating the applicant’s purpose source of rights, supplier, and distributor and shall specify
    whether the items sold are second hand, refurbished, brand new or mixed
  • Certificate or Contract from supplier/distributor/dealer with validity
  • Picture of the establishments facade with permanent signboard (Full Shot of inside and outside)
  • Comprehensive location sketch of establishment with nearest landmark
B. FOR SINGLE PROPRIETOR
  • Valid Certificate of Business Name from Department of Trade and Industry (DTI)
  • Valid NBI Clearance
C. FOR PARTNERSHIP AND CORPORATIONS
  • Certificate of Incorporation duly issued by the Securities and Exchange Commission (SEC)
  • Latest GIS with stamp receipt of SEC or email confirmation from SEC
  • Board Resolution or Secretary’s Certificate authorizing the applicant to sign in behalf of the Corporation
D. FOR IMPORTER, EXPORTER, AND/OR DISTRIBUTORS
  • Submission of notarized list of clients/customers (upon renewal of license)
  • Submission of Summary Report of applied import/export permits (upon renewal of license)
E. FOR DUPLICATORS
  • Notarized Affidavit of Undertaking (for Duplicator CD-Burning or copying in storage devices)
  • List of inventory of equipment/machine/PC Laptop with complete specifications (indicate the Brand name, Model/Type, Serial No., Number of equipment, burner, Disc Writer and USB Port)
  • Photo interior of productions/duplication/replication site
    Payment of registration sticker in the amount of Thirty Pesos (Php30.00) each for disk writer/ DVD publisher/CPU/Tray/USB Port/Certificate/Contract from supplier/distributor/dealer of blank disk and storage devices with validity
  • Summary report of replicated/burned CD’s for the year (for renewal of license)
G. FOR FOREIGN NATIONALS
  • Alien Certificate of Registration, Visa and Passport

  • Please provide a formal letter of intent addressed to Mr. Dennis Lorenzo B. Pinlac, Officer-in Charge – Optical Media Board, requesting if the said items falls under the jurisdiction of this Board. Also submit a detailed brochure with technical specifications. The item will be evaluated and reviewed by the legal division.

    • 2pcs government valid ID
    • Floor Plan (Change of Business Address)
    • Picture of Façade (Change of Business Address)
    • Location Sketch(Change of Business Address)
    • Board Resolution regarding the change of Business Address and/or Corporate Name
    • SEC Registration – Amended (Change of Corporate Name)
    • Updated TIN (Change of Corporate Name)
  • Please provide a formal letter of intent addressed to Mr. Dennis Lorenzo B. Pinlac, Officer-in Charge – Optical Media Board, requesting to change the license category
  • Please provide a formal letter of intent addressed to Mr. Dennis Lorenzo B. Pinlac – Officer in Charge, requesting to be removed from blacklisted.

  • The validity of the issued Certificate and Import Permit is 30 days while the Export Permit is 15 days.
  • The validity of the issued License (Renewal) is 1 year while the validity of the issued License for New application depends on the location as per the Memorandum Circular (MC 2018-003)
  • Please use the Registration number when applying for Renewal of License. You can get the Registration number on the dashboard. Check the previous application approved for License.
